COVID-19 specific:

COVID-19 vaccines are available for free to the public and established FoundCare patients BY APPOINTMENT ONLY for adults and children 6 months and older. Pediatric COVID-19 vaccines are scheduled directly through our pediatric department. A visit fee may apply for the administration of a pediatric COVID-19 vaccine.

History/Mission:

FoundCare’s mission is to provide quality healthcare and social service for all individuals and families.

In 1985, at the height of the AIDS epidemic, an information hot-line named the Information Forum of Palm Beach County (In Forum) was founded under the auspices of Hospice of Palm Beach County with a grant from the Palm Beach County Community Foundation.  Manned by volunteers, In Forum's mission was to disseminate referral and services information to AIDS patients.  

The following year, armed with a four-year $1.6M grant from the Robert Wood Johnson Foundation, the organization became the Comprehensive AIDS Program of Palm Beach County (CAP) offering a wider variety of services to AIDS patients.

Recognizing the need for a comprehensive community health center to meet the needs of the medically underserved in Palm Beach County, the community rallied around the formation of a new federally qualified health center in the county and FoundCare applied for the designation. 

FoundCare began offering primary care services in 2009, was granted Federally Qualified Health Center (FQHC) “Look Alike” status in 2010, and finally granted full status as an FQHC in 2013. It was during this time that CAP and FoundCare merged, with CAP becoming a program of FoundCare.

In 2017, 2018, 2021 and 2022, FoundCare received the Quality Improvement Award from the Health Resources and Services Administration(HRSA), a division of the U.S. Department of Health and Human Services, for achieving the best overall clinical performance and placing in the top 30 percent of health care centers around the country.

Today, FoundCare is the largest non-governmental FQHC in Palm Beach County.

FoundCare accepts most insurance plans, Medicaid, Medicare and self-pay on a sliding fee scale based on household size and income.
